Supporting Documents to be Included with the Florida State University Online Graduate Application ************************************************************************************************ Please note that all items below (and the required 3 letters of recommendation) must be uploaded online. Please do not send your departmental application materials by mail. Applications will not be reviewed unless they are complete. All applications to Graduate Programs in Statistics and Biostatistics must provide the following:  1) The completed application form for Graduate fellowship or assistantship. Please upload this form as document 1 in your supporting documents.  2) A Statement of Purpose (describing your background,accomplishments, and your goals, for example). Anywhere from 1-3 pages is fine. Please upload as document 2 in your AY supporting documents.  3) Photocopies of your official transcripts. Please scan & upload as document 3 in your supporting documents  4) Photocopies of of your GRE scores and TOEFL scores (if applicable).
